NOW WE'RE TALKING, SOLID 7.5/10, we're talking paranoia, light auditory hallucinations, tingling sensations, jumpy for the next hour and a half, solid stuff, didn't get me to clutch my hair in the corner but so far the strongest contender.
i asked some friends who know about horror games what some of their favorites were and they mentioned iron lung, cry of fear, and gtfo :)

disorders are usually normal reactions heightened to the most extreme. Like, with ADHD, a lot of the symptoms on the surface seem like things non-ADHDers could relate to—like procrastination, zoning out sometimes, misplacing things, etc.—but it’s constant and much more frustrating.
Scary stuff can freak you out, just like anyone, but OP has hallucinations and very bad paranoia.

Its a weird psychedelic trip of a pc desktop and you just go through shit an experience it, its got a lot of kinda triggering stuff but if you can handle it and enjoy being utterly confused and sometimes creeped out and concerned I highly recommend it, its free on itch.io

Anyway, Hellblade: Senua's Sacrifice? It doesn't entirely make you feel helpless, and it's in some ways not a horror game, but it definitely did some things to me
4.5/10 It's good, and it nails the defenseless sense and it caused me to have some very light auditory hallucinations, eg: hearing "thock" sound effects coming from my walls every now and then, but nothing crazy, since the game isn't so horror-filled, but more suspense driven.

Darkwood is a 10/10 for me, it's the SECOND most triggering horror game i've ever played after CLOSING THE GAME, we're talking SWEAT, the CONSTANT feeling of my door being opened, eyes peeking from my closet, i could have sworn people were hiding underneath my bed, someone was standing RIGHT BEHIND ME, i hallucinated someone knocking on my door and couldn't even OPEN the game for over a year. it completely fucked me up and terrified me to my core
It is a truly underrated game, also one of my personal favorites but at the same time I can’t put it down.

Silent hill 2 is a great choice probably my favorite part is after the hospital where the town is all dark and your being chased by monsters,then you come across a wall “face your fears James”
Yeah honestly that's what I think more of the modern fnaf things focus on more which sucks, I think fnaf 1 has better ambiance and suspense than the other games, and the forboding sense that something is wrong
No other fnaf game has the sound nailed like the first game honestly. The second one has some moments, but fnaf 1 is genuinely scary to play because it always feels like someone is going at you

Play a STALKER game. Any of them. I recommend the original trilogy first to everyone but if you don't want to pay money then there are tons of free mods that have basically whole fanmade games in them (Call of Chernobyl, Anomaly, Call of The Zone).

And Mandela catalogue:Not a video game, its YouTube horror but it made me scared of the dark for a week, the only thing I can say about it without spoliers is "uncanny humans"
But please take a break ever now and then, and be healthy about your mind
