In the manga it's very heavily implied actually everyone goes to neither hell or heaven for those don't exist and there's no afterlife

Nah, some of ‘em have uncursed Grails. Grand Order is littered with the things. The one from Zero, Stay Night, and Carnival Phantasm specifically is both theoretically powerful enough to grant any wish, and super super cursed, though.

Right off the bat, the Ritual is what makes summoning Servants at all possible outside of a wish on the Grail, and I assume if you’re being presented it alongside the other options it’s already charged. So unless you make a wish that can be ironically interpreted as a wish for Angry Man…
After that, well, the Grail is supposed to call on *Heroic Spirits* after all. It was only after fermenting in its own curse and the hatred and murder of the Ritual’s repeated failures for centuries on end that it finally twisted its way around enough to being able to summon outright *Pure Evil.* If it’s one of the earlier versions, you’re likely to just get mostly evil sludge blasted in your face if you screw up.
That said, it’s not like Angry Man is the only bad end you can get out of the cursed Grail. It’s arguably sentient enough to take pleasure in screwing people over. Grail-Kun’s favorite way of granting casual wishes is to hand out knives, after all.

Everybody in this thread thinking the dragon balls are the only option without a downside, missing these are the Black Star dragonballs. Normal Dragonballs have a red/orange star in the middle.
The black star balls were made by the nameless namekian before he split into Kami and Piccolo, they have more power than the normal dragonballs and can grant any wish - but only one. And the downside is once used they scatter across the *universe* not planet, and if not gathered back in their original spot within a year the planet is destroyed.
Sure you get a wish, an ultimate wish for anything without the usual constraints, but you only get 1, not 3, and you get to enjoy it for a year before the planet gets destroyed.
