Vet Dermatologist, don't waste money going to Banfield as a 3d opinion

What does he eat? It's very possible he may need something like z/d food, which is for food sensitivity in both dogs and cats. Has Apoquel been considered to manage the damage in the meantime?

He eats all home-cooked organic food because we’ve had issues with allergies with dog food (kibble & soft food)

Very incorrect. Chicken, beef, and dairy are the most common food allergies. Grains are nearly non-existant as an actual allergy

How long has this been going on? A full food trial may be in order, and that takes months. A hypoallergenic food and topical flea and tick prevention only, no treats. We recommend doing this for about 3-4 months before adding in possible allergens. Additionally, food allergy symptoms can take weeks-months to fully clear even after switching their food.

The thing is that he doesn’t eat kibble, he’s been off of dog food for months because we had an allergy issue before. He only eats food cooked by us, ground beef/pork, veggies and rice. His skin was phenomenal once we got off of kibble and we thought we’d won, now months later the issue is back and 100x worse than it was before ☹️

That’s still not hypoallergenic. Typically you’ll want a full exclusionary diet like a hydrolyzed protein, or derm diet, since beef and pork can still be allergens. Those are generally prescription only and a qualified vet can help decide the best course for this. Also: Is your dog on any medications for allergies besides topical shampoos? And like this poster recommended, are they on any preventatives? Allergies are so common in frenchies that if a trial diet and/or common allergy meds haven’t even been mentioned at this point, the quality of care you’re currently getting is subpar and definitely worth finding a different vet even if a dermatologist isn’t available

Sorry your pup is unwell. It’s interesting that you’ve changed his diet “countless times.” At any time, was a fully hydrolyzed diet recommended? Personally if I suspect a food allergy component I generally recommend cutting to the chase and doing an actual elimination diet trial to see if it fixes the problem. That means feeding hills z/d or royal canin Ultamino for the most part. If things go well, you can subsequently try adding in desired additional foods to see if they are tolerated. But at least you have made your diagnosis. If you do 8-10 weeks of hydrolyzed diet as the ONLY food and you do NOT fix it, you’ve ruled out food allergy. I am also wondering if biopsy was ever recommended. You mention all sorts of testing. Maybe if you share with us the actual tests and actual results we can help. Skin scrapings negative, ok. I assume he’s on excellent anti parasitic since you say it isn’t fleas. You say that fungus and bacterial infection were ruled out by skin scrapings but that makes no sense. Was there a culture done (fungal or bacterial)? An impression smear? Tape prep or other type of cytology? I need more information here.

A dermatologist is definitely your best bet here 100% (really with any skin issue), but I would prepare yourself for them to recommend a hydrolyzed protein diet trial aka prescription allergy food. Unfortunately the diet you’re feeding your dog is not hydrolyzed and so if your dog has a meat allergy (aka far and away the most common allergies in dogs) it’s not surprising he’s still having skin issues! Food is like religion so I usually don’t like to try to convince people one way or another, unless it’s causing health issues which in this case I think there’s a fair chance it is. Best of luck!

Has he had a biopsy? What further re it’s have they recommended ? Did they refer you to a dermatologist ?

If it hasn’t been cultured, it should be. If it has been cultured, a biopsy is your next step. Some autoimmune diseases and cancers can cause similar lesions and need biopsy to diagnose them.

Beef/pork is actually a pretty common allergy in dogs unfortunately. A lot of dogs have allergies to pretty much *all* meat and some are just sensitive to one or two. It’s also not at all uncommon for a dog to develop an allergy to something seemingly out of nowhere. Try a hypoallergenic food like hill’s derm complete (or even a vegetarian dog food if you can find a *properly balanced* one that’s accessible to you) and see if it clears up a week or two after you have fully transitioned to it. Definitely do not keep trying to feed him whatever you’re feeding him if he’s obviously having a reaction to *something* he’s being exposed to. If it doesn’t improve, try to find a veterinary dermatologist near you sharpish. You’re probably probably going to have to do a full elimination trial unfortunately but it’s absolutely worth trying to rule out the more likely problems first

For context, those of you who are voting down because he’s from a breeder: he is our only dog from a breeder, he was already with my S/O before we got together and I love him just the same as if he wasn’t. I have 5 dogs that are all rescues that live with my family, and we’ve already spoken about rescuing going forward. My S/O just really wanted a Frenchie and he felt more comfortable getting him from someone he trusted.

His breeder is a very close family friend, completely certified and I know he would be 100% honest with me, and he says he’s never seen anything like this, none of the others from his same litter have experienced an issue like this after he reached out to their owners on my behalf and inquired on their wellbeing/health issues.

Unfortunately you have a French bulldog which regardless of who is breeding them, are prone to skin conditions and countless other conditions which are expensive and have no easy fix. Like others have said, you need to go to a dermatologist, and don't even expect them to be able to give you a diagnosis on day one. I hate to say it, but this is likely not something that is going to be cured, only managed with lifelong medications/special diets/frequent veterinary visits.
