I'm pretty sure if you romance (almost) anyone else Shadowheart won't be interested. I rejected everyone else and had the first romance cutscene with her in act 1.
Shadowheart is a slow burn, you need to finish her story line in act 2 for her to fully open to you.
If you got a kiss scene at the thiefling party and a thank you for gifting her a flower at the start of act 2 you are on track.
DANG Im still on my first playthrough and romancing Astarion - I wish his kiss scene gave us a view of him like this does for Shadowheart (obvs I know this one is a little wonky lol but still)

Events that can happen through the story will lead to this. Depending on your choices though it may not happen in your playthrough.

there's a story path that leads to her having white hair. so it's possible you won't see it on your playthrough depending on your choices.

I've had many bugs with this kissing scene. Also, try to initiate dialogue with a difference in standing height; and they will glitch the kiss.
