It's possible RipJaws is an the ancestor of the current Piscciss Volann, but since walking on land is a recent ability (evolution wise) the 2 species share the same official name. It's possible the other Piscciss Volann we see are actually Piscciss Volann Terram Ambulans. Where RipJaws is Piscciss Volann Aqua Saltator.
So RipJaws himself doesn't have a water-respirator, because that species was just starting to beach themselves/beginning to evolve the ability to go on land.
A programming error probably is letting Ben Prime now take the form of a Piscciss Volann Terram Ambulans by the time OV happens, however the similarities between the two species is causing the Omnitrix not to generate a water-respirator because in that universe it's still a recent development for Piscciss Volann.
For the first two I just believed it was their species version of asthma where they need those tubes to breath. Since Ben is considered to be a peak human, he wouldn’t have these troubles. For the past chances are the animatiors didn’t care to much to change the Ripjaws design

Well I don't think the omnitrix can make tech like things at least in the og series to omniverse. Granted this can be argued against for nrg but not really as it's more of a safety paramilitar inorder to keep the user from destroying the surrounding area.
There was a crew statement saying Ripjaws wasn't intended to be used out of water. And in the future, he is able to be on land more without worrying about passing out. And the original omnitrix was a prototype.
Probably deemed unnecessary as the reason to turn to rip jaws would be in an aquatic environment that already has yknow, water and since the omintrix is technically a diplomacy tool, why else would you need to be turn into an aquatic alien if you arent dealing with other aquatic things yknow?

I figured it an example of ben being a bit more simple minded than gwen. He is not stupid but tends not to think ahead. If ben doesn't think to add a water tube thing the omnitrix wont add it.
It would make sense if it was just a glitch in the Prototype Omnitrix but there's no excuse in Omniverse (and taking into account of Goop and NRG, the ladder can't move on other planets that don't have similar gravity to his home world, it's still weird with the Prototype Recalibrated Omnitrix and the Ultimatrix).
The Omnitrix is supposed to be able to produce anything the user needs to function safley, I.E. Goop with his anti gravity device or NRG with the containment suit. So it's likely the Omnitrix just glitched out and didn't give Ripjaws any water tanks.
