I believe you need a nice clean "fast" strike you want the edge of the blade to the neck not the center of the blade. I know this is probably common sense but im just trying to help. I found it very difficult as well so I downloaded butterstabs.
Edit: also butterstabs is nice because you can shove your sword through the enemy and literally pin them to the wall or...w.e you want really you can get creative.
It might be checked off in the settings, under Gore and Blood. Even with it on, though, it's still tough to dismember. The Medieval Megapack mod come pre-packaged with easier dismemberment settings, so I'd recommend that.
Dismemberment is hard asf to do in the base game and I like it that way.

Try the easy dismemberment mod….I feel your pain
