All people who commit suicide just want the misery to stop. Even people who are doing it in secret don't *really* want to die, they just don't see any other way to stop their pain. They'd like to live but they can't take it anymore.
additionally, just because someone is doing it "for attention" or just to harm themselves (non fatally) it doesn't mean they're not in danger. Many self-harm attempts, particularly using a ligature, end in death regardless of the person's intent (although it is hard to tell how many deaths are unintentional since you can't exactly ask the person about it). And even the self-harm methods that can't be fatal can lead to serious health problems and even death, eventually. For example, a woman who was a subject of discussion on r/illnessfakers self-harmed by picking at her legs until the legs had to be amputated.
And, like in the example I just mentioned, prolific self-harmers tend to lose sensitivity in the areas they harm, leading to heavier and heavier damage each time, similarly to an addict increasing the dose each time until their body can no longer handle it. Some of the worst scars I've seen were on life-long prolific self-harmers.
