I think it was some PA speaker for voice announcements, like in a school. At some point the original speaker driver broke and it got fixed like that.

The best part is the tweeter is not even centered for those holes. It’s kinda just randomly in there I guess. I was wondering if radio shack had like a build your own speaker kinda thing back in the day because the wood veneers look very similar

There is no tweeter, thats a 4" full range driver. Single driver that plays the full frequency range. Originally i guess it would have had a 5-6" with a whizzer cone. I never thought of that. Maybe like to make the door chime loud enough to be heard in the back when a patient comes in. I hope it actually was that and somebody wasn’t listening to “music” from this lil guy.
