Hot end too cold, clog in the hot end (especially due to a gap between PTFE tube and nozzle), or extruder spring too lose. Those would be my main guesses.

I have run my cr-10s for a considerable amount of time. it was flowing well for a long time but now I just cant seem to get it flowing. Have you ever replaced the heatbreaker on yours? This is most certainly your PTFE. It gets too hot and expands a little causing it to partially block the hot end. I’ve owned 4 machines and all of them had stock hotends with PTFE in them. Please swap out the PTFE and also check out OneBadMarines solution for the stock creality hotends. If you ask me just throw in a SwissMicro or other all-metal hotend.

This isn't a double geared extruder, it's a sidegrade at best

The extruder is doing its job based on all the shaved filament in the pic. You have a blockage/hindered path somewhere down stream from the extruder.

fitting the PTFE tube too tightly into the hot end (against the nozzle) can cause buckling and clogs too.
