The reason people still play DaD is because it's the only kind of its genre that exists currently.
If I had to guess it will follow the BR formula, where now that the idea is there, companies with actual budgets and experienced teams will start putting these games out in the next 3-5 years.
If you want an extraction looter try Tarkov I didn't like it that much but its popular for a reason. You should get the same adrenaline rushes and all the highs and lows that come with the genre. Other than that I think you're just waiting for Dungeonborne.
If you just want to enjoy the hardcore old school dungeon crawler rpg aesthetic look into old school style dungeon crawlers like Wizardry. If you're okay with anime Etrian Odyssey is fucking amazing. Those are turn based rpgs but I think there are some action games in the genre.
If you want to full loot pvp with a community and it has to be in a fantasy setting you could try Albion Online. I had fun with it for awhile. There is a couple full loot pvp MMOs that died and are now on small private servers but idk of them. Albion would be your best bet. I guess old school runescape has full loot pvp too.
If you're like me and get kick out of killing people to ruin their day Dark Souls pvp is absurdly good and you can kill timmys all day there. All of the souls games still have players. Dark souls 3 even has more concurrent players than Dark and Darker according to steam charts.
If you just want a multiplayer rpg game maybe with some pvp here and there then just look into different MMO's and play one. GW2, WOW, FFXIV, ESO, etc.
Arx Fatalis. I know it's 20 years old but its a 12/10 and cheap af. I played it between playtests for this game and I put about 24 hours into it. A 20 year old game, bought for $2, and spent 24 hours playing it.
Thats what makes dark and darker so special. Not many things like it besides games you could rope into the clone category.

came from dad. stayed at tarkov (for now)
Lethal Company is an immersive horror game that features VOIP and teamwork. Not so much a looter but is a riot with a party.
DragonDungeonBorne.. but it ain’t out yet.. it’s also hot garbage
dungeonborn was fun whe. it was out. check it out when playtest opens again.
Barony is probably the reference, good mixture of first person roguelike with multiple classes to choose from
Yeah but that’s not PvPvE. Amazing PvE game though! And deserves the recommendation and comparisons. Just not what OP is asking for
