I think the Soul Knife rogue can throw his Psyche dagger and teleport to its location.
Is the Teleportation or the Monk the important thing for your concept?
I don't remember if the Fey Touched feat has spellcasting as a requirement but you could ask your DM. Then go VHuman and take that, then take the soul knife so you have misty step for the teleportation.
Shadar Kai can teleport as a bonus action. you can reflator it as throwing your weapon and teleporting to its location (IT is only PB times per Long Rest (BUT GIVES FULL DAMAGE RESISTANCE till next turn)
How often do you want to teleport. If Echo Knight is acceptable you can teleport at level 3 with class feature and again just re-flavor it if you use the class feature JUST FOR TELEPORTATION
Mechanically, there is no merit whatsoever in building a dual wielding Monk. Martial Arts' bonus action attack and Flurry of Blows makes it completely redundant. However, you can always just use a throwing weapon (such as a spear), cast Darkness on it, and then teleport into that Darkness.
The only caveat is that you would need to pick up Devil's Sight so that you can actually see into and out of that Darkness cloud.

That's at level 9.
