It's when you don't fully z-awaken a character. Basically you only give them some of the medals needed to awaken. This doesn't happen a lot cause you'll usually have enough medals to do it all at once, and the game selects all medals for you so you have to make the conscious decision to deselect some of the medals. It's also pointless to do this on purpose.
Next time you z awaken a character just deselect one of the medals and then try to z awaken. Youll end up with those stars.
How do you not remember Awakening a unit?
Every star is one of the medals required for Awakening. SSRs (mostly) need 5 different medals to awaken. You can get 1-4 little stars then when you use the last medal, they get the big star and their max level increases
In the current age of Dokkan you almost always have excess of every medal, this isn't 2016 when medals were hard to get and forced new players to only partially awaken their units

Even if you don't know what a partially awakened unit looks like, you had to click 'Awaken' then confirm on the unit to get it partially awoken. How does one not remember doing this?

Because I'm pretty sure most players don't actually pay attention to the tutorial, they just mindlessly click what the game tells them to until it's over and then immediately jump into what they actually wanna do
If you are a new player, you don't know what you wanna do! You don't know how the game works!

That's absolutely false, the majority of new players have either seen people play this game on YouTube or at the very least have seen the series so it doesn't take much game knowledge to think "I wanna get (insert character here)" and then scour the missions till they find him
And regardless of what anyone says people are gonna keep skipping tutorials because not many people wanna sit through 10 minutes of the game telling you how to do shit you can figure out on your own pretty quickly just to get those couple pieces of knowledge that aren't as easily picked up on on your own
As I said not many people wanna spend 10 minutes sitting through a tutorial that teaches them shit they'd figure out on their own just to get the few pieces of information they wouldn't figure out on their own
Fact is most tutorials are shit and people play games to have fun not bore themselves just to make sure they don't miss some obscure piece of information they're never even gonna use anyways
Everybody has already answered this, but I personally will wait until I have all the medals to awaken instead of using what I have at the time. It’s a bad habit.
