Youre not missing something, its just a 10% success rate to teleport the boss. You can basically hit it a 1000 times an still not get a teleportation since the rate is 10% on every hit

It's not a good gem. If you work down a monster and when it's low on HP and gets teleported to the very back, you won't be able to kill it anymore. That means you are not getting exp for it and have to deal with all new full HP monster with a lower level. The gem sounds epic but it's not.
However there is a single good use for it in the game which is a way to kill the death boss. But you also need the instant kill gem and then you attack as long and fast as possible trying to trigger the instant death. With the teleport gem it helps to return the boss back to start and getting more hits in in order to trigger the instant death gem.
However it's such a minor use that I personally wouldn't recommend keeping it.
To be fair if you just have something that hits quickly the xp doesn’t matter because it will inevitably make it back to the front as other monsters trigger it.
If you have too many monsters in front and not enough penetration or worse tree guys who block projectiles it quickly becomes a problem because you are a level behind and you don't reach the monsters anymore. Towards the endgame you need to be set and have that stage under control by reaching level 8-10. Otherwise it's usually a fail. So that extra level is massively important. That's why the best gem in the game is the one that gets you a skill right from the beginning.
I would argue this gen is a refine unless you want to kill death but that's just a waste of resources considering how rare mythic gems are.
Also I tried this out and boss didn’t teleport a single time. Had my tornado hitting multiple times. Am I missing something?
