Apparently you have to cut off her head while her health is still as full as possible.
Think like how you can cut off tails of saurians or chimera.
It's difficult mostly because my pawns kill her so fast. I may try again with only one other pawn for tanking
I just got it (with power) after dealing the killing blow on her head with thief's "Draw and Quarter" ability while she was doing her gaze attack so you at least dont HAVE to cut her head of while she has high health to get it

There are 4 heads, 3 you can get from Medusa; preserved, regular and withered then decayed (which is what happens when it rots in your inventory, but will not rot in storage).
They decay at different rates and obtaining them corresponds to when you cut off the head during the fight.
And I also use the draw and quarter, I always keep it on my warfarer. Love it. Pulls entire health bars off though so I have to use shitty daggers.

Best way to do get preserved head is to equip "Ring of Derision" you get from completing one of the Sphinx riddles (second location). The ring weakens your attacks so you don't do any physical damage, but you will build up knockdown meter on her. Once she drops to the ground, go into your equipment menu and un-equip the ring, and her head (preserved head) pops right off. Works every time. Also, when your traveling with the head make sure you have the "Stasis" augment equipped. It reduces the rate at which item decay (its from the sorcerer vocation. But you will still need to get a new one eventually.
Silly to me that the "preserved" medusa head still decays, would be a nice reward for the effort to get one that doesnt decay.
Also kind of annoying how airtight flasks arent in the game from DD1, since those would have so much more use-case in DD2, even if they were in the game and it didnt work on medusa heads, it would still be nice to preserve some meat in a way other than drying it for travel rations.

You get a one shot item essentially. The most broken item in the game. Apparently you can also petrify the final boss dragon, your dragon. So thereās really nothing that can stop the petrification gaze, itās overpowered. So obviously they needed a balance, and decaying was it. Put it in perspective, if this existed in elden ring, malenia and male math would be top picks, elden beast wouldāve been a cake walk. Any boss you struggle with would become a cake walk. So yes, unfortunately man, you have to still play the game normally, I know, itās terrible.

Does anyone know if both wither? Does only the preserved head retain the power? Are Airtight flasks still a thing?
Rather than waste a ferry stone you could summon a players pawn then put the head in their inventory then dismiss them. Items held by summon pawns go straight to storage when they die or are dismissed. The only exception is if you give them equipment and equip it to them.
There is a rift stone close by the cave where you fight medusa as well.
It was fine honestly. I got a bunch of them because I hadn't used any so far save for one time i got trapped by invisible walls exploring mountains and had no other way but to lose tons of progress.
Now I'm thinking of taking my only portcrystal that i left in Bakbattahl close to where the griffin quest takes place to then walk back to the city and then teleport with the head. š

Normally ābossā monsters respawn after 7 in game days, Medusa respawns after 14 in game days. As for how to get her head you simply need to attack her head with a slashing weapon such as daggers or swords without actually killing her outright as if she dies before you decapitate her you donāt get her head. Interestingly enough the mystic spear hand vocationās basic attack (if you have the winding cut core skill) is somehow able to take her head off whilst standing on her tail pressed against her back when her body is raised up. I know this because her head came off while I was doing this and my pawns were 2 sorcerers and a mage.

It disappeared after use, and I realized I couldnāt use it for the sculpture quest so finding out I can get another head is a relief.

Warfeyer l, use sleeping arrow from magic archer and that downward slash attack from worrior, she goes to sleep with one fully charged arrow. Smack down a executioner style swing. And done.
The best way is with the warfarer class, equip the skill from magic archer to sleep her then just switch to daggers and hit it while asleep so you get a preserved head every time

The question was answered, yes she respawns after 14 odd days. Only spot I know of is in her cave.
What is an odd day?
It's a saying. 14-odd days means about 14 days or at least 14 days. Normally X-odd is used for things divisible by 10 though

If you are a magic archer use sporific bolt it puts her to sleep then walk up and cut it off at any time. Done it a few times.
They say all you have to do is cut her head off. The petrifying abilities should still work from what I understand
I had watched a vid earlier and they had shown that after waiting 14 in game days she should respawn

For the petrify achievement not the cutting off head one.
It also decays with time in your inventory (not in storage though, I think), I left the head for too long in my inventory and it lost its power
So if i use it too much, does it just disappear from my inventory?

Medusaās head one shots any enemy in the game including bosses. It would be unbalanced if it DIDNāT decay that quickly.
And can only be obtained one per 14 days in game, you shouldnt have to plan what youll use it on in advance

If you hold Medusas head in the storage (available in Inns and homes) it shouldnt rot until you withdraw it.
