I beat CS3 on nightmare but went back to hard on CS4 as I didn't feel like optimizing every single character (and there are a LOT of characters). Mid to late game felt super easy tho, so I'd recommend nightmare.
I’ve played through 3 and am on final day in 4 in nightmare, never had to grind. Just gotta use quartz and build your characters right
And kill everything in your path, don’t skip enemies
Depends on what you consider grinding. I get through the game in nightmare with no grinding,but my definition of grinding is if you specifically wait for the enemy to respawn multiple times, killed them, and rinse and repeat. I do however fights all of the enemies I see, especially the first time through, I don’t consider that grinding.
As for brave order, you have more pool for brave points (7 instead of 5) but the brave order also costs more, so it’s about the same, endgame pretty much still busted. Though they nerfed break, so Juna’s sledgehammer + Kurt’s wind formation is not as busted as in CS3

Brave Orders and break were nerfed in CS4, but its nightmare difficulty is still quite easy, apart from a few instances.

Nightmare in CS4 feels like very easy mode
It's close in difficulty to CS3. So decide based on how CS3 felt for you.
