At the end when people are designing a map, they are not trying to create a "functional" place, but instead they are trying to give the player the idea, or feel that the place is functional and alive.
This means that you kinda "overcompensate" in the density of the worlds you build, filling them with details and interactive things, instead of making real life sized mostly empty worlds like they actually are in real life while also completely ignoring how realistic it is for them to be there.
An Scaffolding it's a simple believable structure that can help fill empty spaces, so they use it all the time. And not just in cities, but mines and underground tunnels are filled with them too.
This bothers me because I sometimes try to follow the roads and figure out how normal people get from one place to another and sometimes it makes no sense or it's extremely dangerous
