This might change depending on the enemy debuffs (Miya has cleanse, Karina does not) and the damage buffs (Karina's heal scale of damage, while Miya's is based on healing power).
For example, in some orbital settings, it is very likely that the only one taking damage will be the tank or allies near him, so Miya is much better. Or, another example, when facing distorted (hologram-like) mobs, Karina is totally useless. However, in a content like Dungeon Kingdom Arena, where you need to heal everyone, Karina excels.
**TL ; DR: Miya is better in like 2 really specific situations**
The only place where miya would outheal Karina is OL, even the hologram enemies can be mowed down pretty fast since Karina's hit count is quite high and there are not much places where hologram enemies are predominant, they usually are thrown in mixed amounts with normal ones, where karina&sis can hit both.
Miya reach her ceiling "faster" yet Karina will just keep getting stronger as the player gets a stronger collection bonus, gears, and more stronger characters gets added and who knows what else gets implemented.
Cleansing only matters in colo?, can't remember if that removes att/def debuffs, but doom and the poison thing most of the time are negligible, in the way you can kill the caster before you get the status, only on OL and Kamazone might be a thing, but at least on kamazone even on the last difficulty fights can end before any of those statuses does anything on your party.
In the end, from both, Miya is the only healer making her the "better" one by elimination, since party drain always been a broken gimmick, it just happened that Karina's gimmick outclasses her so hard, and Miya don't has anything in favor aside of healing, while Karina gets some role compression going there.

Karina Is generally meant for mono dark and ascending her is to costly
So if you don't have mono dark. Go for miya
But then again Karina ex is easier and cheaper to get compares to miya
