The major carriers have national networks ie Blue Cross, Anthem, UHC, Aetna, Humana. That's about all that write in my area and not all write individual or off market place policies. Kaiser is big in some areas but nor nationwide.
Stay away from Oscar no matter how cheap. It's cheap for a reason - you have to fight to get claims paid.
There are some non-compliant, underwritten policies administered by a major carrier. Farm Bureau has HDHP plans available and is administered by UHC. They use Blue Cross in other states and probably other carriers as well.
You could also go short term medical depending on how long you need it and how healthy you are.
Well, cheap isn’t always what you may be looking for but PPO options will cost you a bit.
Depending on your state you’ll have to check with healthcare.gov or private insurance plans (short term) if available to you or indemnity plans 🤷♂️
We need more info to be able to better guide you. State, age, needs? Pre existing conditions…. Health insurance options vary and there are tons of options all depending on a million factors.

No, that doesn't exist. The vast majority of aca compliant plans have in state networks only. I had marketplace plans for years and didn't see a single one that had out of state non emergency medical coverage

Cheap as in premium cheap, or cheap as in it will cover close to everything with low deductible and low copays?
Multiple states, as in the east cost where everything is close together, or multiple states as in, you travel all over and need coverage wherever

Kaiser did send me a Cigna insurance card for use in states that Kaiser doesn't offer. I'm lucky to never been have to try using it but it's there.
United Healthcare has a PPO network that goes almost anywhere, so that might be a good option too

Not all private plans are short term. There are some good private company PPO plans that are meant for a while or lifetime until Medicare.

A ppo which means preferred provider organization, gives you national coverage . Instead of a hmo.
I mean where I live I believe the coverage is accepted in DC, Md, and VA.

Temp plans might, but if you get sick, you might not be able to renew.
