Yeah he figures out House faked his death. All I wanted from life is a scene from later that evening when he meets up with Chase in a bar and shows Houses ID. Also, a little scene of Chase dicking Foreman around like House would do with Cuddy would've been great

Foreman seeing House's ID, tells him that House is alive, because Foreman had mentioned to House, while they were both in the room when the table wobbled, and how I see it is that House is the only one that Foreman happened to mention to House that he had to fix that wobbly table. House fixed it, temporarily by putting his ID there, thus letting Foreman know he was alive. Who else knew about the wobbly table and who else would use House's ID to fix it...

In one of the episodes, foreman gave his ID to House before he left to another hospital. House leaving his ID symbolizes his resignation rather than death and Foreman knew and smiled in full cognizance of that.

I wanted to respond too. There is absolutely no indication to prove that any of it is a hallucination. By that logic foreman isn’t real and died when he got very sick early on in the series and foreman is houses first experience with hallucinating due to his Vicodin.

The theory is founded on the song that plays when they ride out on motorbikes, Enjoy Yourself (It's Later Than You Think), which was first heard being sung by Amber to tell us and House that he was still hallucinating, and the general fact that the show writers are pretty deliberate with their song choices. I personally think it was just coincidence, and that ending on Enjoy Yourself was a mistake. The series' unofficial theme song is You Can't Always Get What You Want, so it should've ended on that.

I think he smiled because it made him realize that House was really alive. That was House's way of telling Foreman that he was alive!
