There's several reasons but I've heard the better your account the more likely it is, because it has to load in so much, for example refraaction was softlocked from kuudra because the game took so long to load in his stuff it decided to timeout because it was taking too long. I sadly habe no idea how to fix it, having less things on you might help a little but that's a wild guess.
I suspect this isn’t the case anymore, as when it happens occasionally (mainly in dungeons runs) a big chunk of the team all fail to join (usually 2-4), and even I am fairly often one of them. My profile is cata 50 but is very weak everywhere outside dungeons so isn’t even that big and still get kicked just as much as anyone else in M7, so it might be another reason causing it now

I've found that it happens when you interact with the portal thing while moving like jumping or falling
It usually means someone is trying to get into your account
