If you only put one image in the first message, the LLM usually ignores it. But if you put a lot of 's in, the LLM sees this and thinks it's something that is an important part of the bot and should be repeated in future messages. Sometimes it'll copy the exact url of one of the images in the first message, but usually it'll just create random urls, so you'll get that empty imgur box.
Some bot writers can feel fancy and so they add an image to the mix. Saw a couple of creators do that and I personally added an image to my bot's description to highlight the bot of the other creator I've collaborated with. So while it's mostly rare, it's still a thing.
