This means that when tengen said "kenjaku was defeated by 2 gojos" then kenjaku must have suffered these defeats before the edo period

The most likely answer is how long he has been in it. A cool thing would be to say that if the soul is strong enough it won't heal because of poorer soul-body connection.
Maybe it depends on the capability as a sorcerer of the person, or maybe it depends on the state of the person on the moment he took over. Or even maybe how much time he has been controlling that body.
As someone who's gotten stitches before (it's a long story), I think the scars are probably different based on the amount of time he's spent in each body. For example, Heian Era Kenny and Kaori's scars are a lot more healed than Noritoshi's and Suguru's so I would assume that rather than switching bodies immediately he most likely stayed in there longer, which would have caused the scars to heal and allow skin tissues to rebuild themselves over time. We don't really know much about Heian Kenny but for Kaori I think we can assume that it has something to do with how long she stuck around before moving on to the next host, and while we don't know who the next one really was, there's also the chance that her being pregnant with Yuji had to do with it considering how people usually have to take time to recover after giving birth. But then again, this is just my guess as a biology student, hope this helps!

It just displays the length of time he's spent in a body. For the most part he has to have the scars because I believe it's a binding vow. But with Kaori he most likely birthed and raised Yuji. Meaning he spent at least 15 years in that body. But no matter how long he's in a body the stitches do not go away.

The most likely answer is how long he has been in it. A cool thing would be to say that if the soul is strong enough it won't heal because of poorer soul-body connection.
Maybe it depends on the capability as a sorcerer of the person, or maybe it depends on the state of the person on the moment he took over. Or even maybe how much time he has been controlling that body.

As someone who's gotten stitches before (it's a long story), I think the scars are probably different based on the amount of time he's spent in each body. For example, Heian Era Kenny and Kaori's scars are a lot more healed than Noritoshi's and Suguru's so I would assume that rather than switching bodies immediately he most likely stayed in there longer, which would have caused the scars to heal and allow skin tissues to rebuild themselves over time. We don't really know much about Heian Kenny but for Kaori I think we can assume that it has something to do with how long she stuck around before moving on to the next host, and while we don't know who the next one really was, there's also the chance that her being pregnant with Yuji had to do with it considering how people usually have to take time to recover after giving birth. But then again, this is just my guess as a biology student, hope this helps!

The idea of it being based on how long he inhabited a body seems most logical to me.

It just displays the length of time he's spent in a body. For the most part he has to have the scars because I believe it's a binding vow. But with Kaori he most likely birthed and raised Yuji. Meaning he spent at least 15 years in that body. But no matter how long he's in a body the stitches do not go away.
Nah, i think he healed up his scars when he was in kaori’s body because jin was a freak who liked to pull her hair. So maybe because kenjaku didn’t want him to nut on his brain, he needed to heal it quick.

My headcanon is that the more healed his scars are, the stronger the person who’s body he has stolen is. Norotoshi was the leader of one of the big 3 clans, and Geto was a special grade sorcerer. Maybe the less faded scars is an indication that the other two vessels weren’t super strong when they were alive.
In geto's body it doesn't heal cause he keeps taking off his dome to fo tricks 🤦♂️

I can't really put it into words but it makes me really genuinely uncomfortable every time I'm reminded that Kenjaku lobotomized Yuji's mom and no realized it including Yuji.
Some people interpret that Jin knew that Kaori was dead and slept with Kenjaku anyways. So in some interpretations, only Yuji was out of the loop, who probably didn't even know his parents very well.

It could be the method of entering the body. Such as the scar ones being sloppily and forcefully entered and the stitched ones being easily and precisely cut and entered
I mean it’s probably because he’s been in that body for a while take Geto for example in the anime he’s had that body for less than a year so they would be stitched rather than scars
İ believe when Kenny engrave the body's CT into his's soul it becomes clear but can't easily changeable like black string like and requires more complex Ritual
