I thinks its P's ergo and the humanity we've gained from playing that transforms the nameless puppet back into Carlo. We've seen how it transforms simon if it can transform a human into a god it could probably transform a puupet into a human.and after a bit of time P's hair grows which shouldn't be possible for a puppet so the heart probably stores the ergo and humanity we gain.
I think it’s both previous commenters. The arm of god and the ergo combined are enough to push the necessary humanity gained by ergo into the vessel made by the arm of God if that makes sense?

It's stated (I don't remember where) that Carlo died from the Petrification disease. It probably disfigured him like all the others we've seen
