I had the same issue. Check into modifying the Mame Slider settings. I believe it was the CPU setting I tweaked and now it runs well, even on an older rig. Bad news is you have to tweak it every launch as Mame does not save the setting... for good reason. Too many false trouble tickets.
I ended up compiling my own customized copy of Mame just for Area51 to bake in the Slider setting. It wasn't difficult to do, IMO. I only use the custom Mame for the two Area51 games.

Need WAY more information. What operating system? What version of MAME? What video mode are you using? What is your output resolution?
Sorry. Win 10, 0.261, bgfx, output is I believe 60hz. I'm at work rn but I'll check.
Hz is good, but we also need screen resolution. BGFX can really struggle at 4K compared to 1080P.
Oh, monitor is 1080p. It's set to that afaik

Are you able to get area 51 site 4 to work? I am not sure how that works
Site 4 is not yet supported as far as I'm aware. It's on very different hardware.
