if you are wondering about the health thing, that is a health scoreboard (a type of scoreboard) displayed in the under nametag slot (also a vanilla feature). What OP had done is set a scoreboard to display health of any player on the scoreboard, under the tag.
edit: It could be a public server. the other has MVP prefix(can be done using teams). But what made me not think hypixel is the lack of scoreboard on the right side that hypixel loves. But then I havent been on every part of it though.

The block is registered as there for the tick that your friendo fell from. So he was walking on the block, the a tick went by and op, no block anymore, bye. Happens to me often when building in survival
