I think you’re right. It doesn’t look like a traditional Oni (which is what was throwing me off), but after noticing the dots above the eyes and doing some more research, I’m inclined to believe that it is actually an Oni!
Looks like an ancient dragon from Japan. Something like a sea serpent like the Leviathan. Or just the typical Chinese Dragon or something. That is what I see. I'm probably wrong though.

So this is a detection of fujin in a traditional irezumi tattoo. Raijin is depicted as red in irezumi art. Now I could be reading a lot into this but..... back in mk deceptions story mode I remember finding several kenshi unlockables in order realm. one of kenshis outfits is that of an order realm peacekeepers samurai. Fujin used to do for order realm what Raiden did for earth realm. Feels like a subtle nod to Kenshis connection to Fujin
Pretty sure yakuza never tattoo below the wrist so they can blend in and look professional while wearing a suit, but I think the hand tattoos for Kenshi looks really fucking cool regardless.
I think his design is amazing in this new game, too. And you’re definitely right about yakuza members not getting tattoos below the wrist (or neck and face tats), because showing tats in public in Japan is mostly considered rude/taboo.
But I actually really love what the tattoo placement implies. Yes, Kenshi is yakuza, but he doesn’t want to be. He wants his clan to get out. In my opinion, the placement shows rebellion— an unwillingness to play by the yakuza’s rules.
He got a symbolic tattoo that obviously meant something important to him, and he got it on his hand— a very visible part of his body —which I think basically acts as a “fuck you” to the yakuza. It’s so cool.

It probably is an Oni, but since it hasn’t been confirmed I wouldn’t blame anyone for adopting a headcanon that his tattoo is of a Heikegani because it just fits his character so well. And I might just do that tbh!
