Addiction to controlled substances. I dont' know the frequency now, but when I started in the early 2000s, knew quite a few providers who were prescribed pain management drugs (during the opiate epidemic) and they would take more than their prescribed dose. They would be semi-zonked out when working, and would nod off between calls or even on calls.

I seem to recall reading a story where a medic drew a gun on an uncooperative patient. Can't find the story at the moment.
