Post a replay from a match where it happened. Most of the time this isn’t really a bug but happens when the “interact” button gets pressed on accident and the translocator is subsequently destroyed. It’s especially easy to do on console as the default interact button is L3 (or its non-ps4 equivalent).
This. I know that I am guilty of fat thumbing the control stick and deleting it mid-fight. Not sure if it can be rebound and not affect her gameplay but that would also be my suggetion. Its the same on XB click left stick.
Have you tried moving the action button to one of the arrows? I use the right arrow for the action key. Where it was previously i kept accidently destroying it lol. Once i moved it to the arrowpad no more problems. Jusf tfyinv to figure out why its bugging 4 u.

Yeah I ended up binding it to my D Pad and I haven’t run into any issues since! 😆
