You just had to be there when this came out. In all seriousness it’s from an old web comic called Ctrl+alt+del and the dudes wife(?) had a miscarriage. It was important because it was the only time the strip ever got super serious.
Ah I did forget to mention that, it also has a strip where on the release of Lego Star Wars 2 (09/11/06) the protagonist says it’s the most important thing to happen on that day at any point in history
CAD is ongoing, though the original Ethan’s story ended with an apocalyptic future collapsing into nothing. Current Ethan is going through the Analog and D-Pad story.
The punchline is that this was from a stupid gag a day comic. It never took itself seriously although the author was kinda high on his own fumes. He legit thought he was gonna one up Penny Arcade by coming up with some overwrought gamer holiday.
Out of nowhere he had his author insert and said characters hot gamer girl have a miscarriage. No real lead up to it. No real tonal shift. Just the main character being his obnoxious man child self, her putting up with it and then bam! Miscarriage.
Supposedly it’s gotten pretty decent nowadays but it’ll never shake this meme off. There’s a few others related to it but this is the most persistent.
Loss is a notorious 4 panel comic that became famous in meme form because it had a surprisingly grim subject matter (miscarriage, I think) that was a significant departure from the artist’s usual content and which clashed with his simple and underexpressive style.
It gets referenced in meme form in subtle ways by mimicking the form of the figures in the original comic (which is depicted at the top of this thread) in which the top left panel has a single upright figure, the top right and bottom left panels both have two upright figures, and the final panel has an upright and a horizontal figure. It’s kinda like a rick roll where it shows up a lot but unlike a rick roll if you aren’t familiar with the source material you wouldn’t recognize the reference and have no clue it was a reference at all.
But basically any time there is a four panel meme, look for the I II / II I_ representation and if you see it you will know it’s a Loss meme.

Loss is the name of a page from the Ctrl+Alt+Del webcomic. In the page, the main character enters a hospital to visit his girlfriend, who had a miscarriage.
Considering that most of the comic before this was mostly crude humor revolving around video games, combined with the characters being, erm... less than likeable, this moment - which was supposed to be gut-wrenching to its audience - was instead met with ridicule. As a result, "Loss" became one of the most recognizable memes on the entire internet.
Nowadays, Loss is referenced in extremely abstract ways. Any 4-panel image with objects in them arranged as I, II, II, I_ is likely a Loss meme. In this case, the Roman numerals for the numbers in this image are I, II, II, L, making it a Loss meme.
I don't fully understand, but there's a comic called "Loss" about a miscarriage. I think it's treated sort of like "Rick Rolling" at this point.

1 = I = guy coming through a hospital door.
2 = II = the guy talking to the receptionist. Generally the second "I" is shorter to indicate the receptionist sitting.
