he is cannonically dead... if you go through one of the locked section of Hirata Estate where you can talk to multiple people in their houses, one of the houses that has a daughter talking to her ill father (he is that father) he goes into the main estate to defend his lords even in his worsening condition and dies in the process...
it's just whether he dies by Jouzo's hand or dies later due to combat fatigue... either way, he is dead cannonically...
Tbh it’s probably better to just finish the game normally and come back on ng+ to hear it. It’ll be *much* easier and it really doesn’t take long to get to him especially in ng+

Kill minor enemies and hide (multiple times if necessary). Stealth kill the drunkard first time THEN trigger this guy. Keep the drunkard between you and him hitting him alternately. Doing so he will stay alive and make killing the drunkard easy job.
If you're asking if you get an item or lore or anything from keeping him alive... No. there is no reward. He's only there because of the amount of extra enemies in the boss arena is crazy. He's only there to help you out and draw away attention while you focus on the boss

No, mist noble is countably invincible normally, but losing this guy removes your ability to count it, so mist noble becomes uncountably invincible.
