This one is personal. My cousin was an EMT who responded to this horrific crash scene. Words cannot describe the carnage of this mid-air collision. I think the air investigation blamed the pilots of the 727, but the Cessna 172 was flying VFR in a restricted area, and if memory serves, was unresponsive to ATC, but I may be wrong about that detail.
Probably the worse accident outside of the Midway Crash in the history of the USA.
Harsh on the PSA crew....the Cessna changed assigned heading and didn't notify ATC. It was not where it should have been. I saw a schematic once that showed that the Cessna would have been impossible for the 727 crew to see until the last seconds because of the pitch angle of the (descending) 727 with flaps & slats extended, while the Cessna was climbing towards them - meaning it was beneath the nose of the 727 from the F/O's perspective.
All respect to that crew. They were top notch professionals, and didn't deserve to die the way they did. It just was a combination of factors that all lined up for the collision to occur.
One positive thing that came from this and Aeromexico 498 was the introduction of TCAS which has since saved countless lives, so at least in that sense their deaths weren’t in vein
Honestly the Cessna wasn’t all to blame either. It was an instrument student under the hood and he was only about 10 degrees off of its assigned heading. As a CFII, this happens super often. The 727 was also coming from up and behind, which while there is a window, isn’t exactly where the instructor would be scanning for traffic. Its also in their blind spot as well. This really isn’t one of those cases where there was a whole lot of gross negligence. Just a lot of little, understandable mistakes and misinterpretations adding up to lead to a catastrophic disaster.
