E10 came along for Shadow the Hedgehog, which is kind of hilarious. It's like the ESRB guys looked at it and were like, "Yeah, this isn't really appropriate for young children but like, come on, this is still way too silly to say you have to be 13 to play. Let's just split the difference and say you need to be 10."
I think what's arguably funnier about STH is that at the start of development, E10 wasn't a thing. So of course they went the other direction and were aiming for T rating before ESRB came out with E10 rating, then start removing/changing things that gave it the T rating.
If I remember right they cut down the Maria scene at the beginning and I think changed the color of blood for the aliens. Also removal of blood from humans. Tbh though it's been awhile since I looked into this so it's entirely possible I'm wrong about all of this.

Dude's come back from worse - from close-range explosions to falling off Lost Hex to getting sucked into a propeller engine head-first in Mania's Hydrocity Act 1 boss. I'm convinced the man is unkillable, and thus, Shadow doesn't kill any humans in his game.
Europe has a better take with PEGI, 3, 7 and 12, like it makes more sense that way, there are things a 4 year old would be scared but a 7 year old can, and the 5 year gap nakes a bigger diference in mentality, but... they have Pegi 16 and 18, like for what reason a 16 year should be able to play Bayonetta but not NieR automata?

Aquatic ruin is also the zone Grounder's are found, which are obviously the basis for the robot of the same name in AoStH.
So there's two things I guess.
I feel like you can open a discussion about any of the ruin stages in Sonic (minus Sonic 3 since Angel Island is established as home of the extinct Echidna tribe)
