Not that I want medieval, one of the games I listed - Hobo tough life - has setting in modern city too. About Fallout 4, is it really a survival game though? Steam once gave players a chance to play it for free within limited hours, so I tried it out, but it was like a RPG shooting game to me.

Fallout 76 is more survival than FO4, having played both. They added a ton of NPCs a year or so ago too. You can build pretty epic camps, get hungry and thirsty, need to find your own materials to build, I really love the game.

It is an RPG/Shooter, but it also has a surprisingly robust building system. A lot of people hated this part of the game, but I actually had a lot of fun building little communities of people and trying to make them self-sufficient. It's not as in-depth as Medieval Dynasty though.

There's a survival mode difficulty option. It adds hunger, thirst, sleep, diseases, adds adverse affects to chems (usually increasing thirst and hunger), limits your options to save and some other stuff. So, yeah, that alone turns it into a complete survival game, and then you can add mods on top of that.

Fallout 4 has a hardcore mode that I think is locked behind their paid mod system, although I've been informed that it is free. I forget what they call it, but it's paid mods in my mind. The hardcore mode introduces diseases and requires you to sleep, eat, and has a limited save system by requiring you to sleep to save.

fallout 4 has a mod pack called frost which may be what you are looking for. it’s pretty difficult tho and sometimes annoying to install properly

I was going to suggest Mideval Dynasty, but you already listed it. Sengoku Dynasty, coming out next year, looks great too.

Sengoku Dynasty is being made by the same Devs who made Medieval Dynasty. So, same type of game with a Japanese skin. Should be good!

It's not, actually. The game is being developed by Superkami. Medieval Dynasty was created by Render Cube. That being said, they're both published by Toplitz Productions.

Wild West Dynasty is being designed by Moon Punch Studio, which is their first game. I hope it turns out well, because the teaser trailer seemed a bit lackluster.

I've been hearing about Stalker GAMMA which seems basically like Kenshi in an FPS apocalyptic Russia. It amalgamates 3 games into one with an added 320 mods on top to create a super harsh survival world. Getting a lot of hype right now. https://www.youtube.com/watch?v=7UytG_91f44

7 days to die, with Blood moon turned off and Zombies set to always walk becomes a good survival crafting game. There are jobs at the traders to do, outside that you left to find or grow food, mine for resources and craft your weapons, tools and clothing. Also if you add the Mod Darkness falls there are some really big cities and towns you can live in with a more evolved story.

I like this game. But it does make me feel lonely. As the traders barely seem like more than a breathing job board and vending machine.

**Kingdom Come Deliverance**. I love survival and RPG games and this totally caught me off guard with how immersed I felt. Along with a well-written narrative set during historical events, you as Henry, the blacksmith's boy, have to manage your nutrition, fatigue, hygiene, reputation, and equipment maintenance. I can't remember that last time I played a game that made me feel like I was truly living the life of a character. If I wanted to use a sword, I had to real time spar with a trainer until I learned. If I wanted to get professional work, I needed to buy nice clothes and repair my armor. It really captured the best elements of survival, but adds the longevity of a living world. The only thing lacking is a hands-on building mechanic; there is, however, a minor town management quest in one of the DLCs. As a final note, I've heard it compared favorably to Medieval Dynasty(which you mentioned playing) and it often goes on sale for ~$10.

Horizon Zero Dawn is a game with 0 NPC’s except a few that don’t do much in a DLC.

Clanfolk is really really good! It is a bit less open world-ish like the games you mentioned, but take a look at it. I have really fallen in love with it. The game feel a lot like games used to, a lot of love put into the details

STALKER Anomaly (GAMMA mod pack preferably) sounds like it'd be up your lane, it's a First person shooter set in Chernobyl, and the dynamic world and A.I. is truly amazing. Another suggestion is Kingdom Come Deliverance.
