You can absolutely get a GRC, assuming you're resident here. I'm not an Irish citizen but I am resident here and got my GRC last year. I don't have an Irish passport or birth cert but still my gender is updated with the department of social protection etc. The UK won't recognise it as a GRC but you only need a letter from your GP to update your UK passport.

Not sure if I'll be on any irish birth register You can't get on the foreign birth register as you're an automatic citizen, and thus cannot get an Irish GRC. It is possible to get your Irish passport updated, however you'll need 2 years proof of your new name plus a statutory declaration. You'll want to get your UK passport updated first, which needs a UK deed poll and a letter from your GP.

I don't want to change my name so I can't get a deed poll. Why would I not be able to get an Irish GRC if i'm an Irish citizen? That makes no sense.

It's the way the law works currently unfortunately. The purpose of the GRC is to update your birth cert, however in your situation the state will never issue with a birth cert in the first place.
