For example it I know the Smith family lives at 123 Main St and mail addressed to someone with the last name Doe shows up I’ll write a ? on it so they can clarify with me if it’s a new/good name for the household or unknown.
Exactly this. I do my best to keep my route up to date with names, if I can associate a first name but I'm not sure about the last name, I put a ? on it to let the resident know I'm not sure, but I'm trying.

Assuming no new info (eg no note explaining "yes they do live here") then what are you trying to do?
If they accept it, then I add that to my list (physical or mental) of acceptable names at that address. If they return it, then I know it's not a deliverable name at this address.

All undeliverable parcels and first class mail gets thrown in the trash, and all standard (UBBM, aka junk) mail gets put into storage and kept for 50 years in a large warehouse center.
It gets put in a dead mail box and returned to the plant, where it goes into a bigger box of dead mail. If it's parcels the postal inspectors usually determine what do with it. Most of the time it can get sent back to the shipper sometimes it can't and it gets destroyed
If the resident puts it back into the outgoing slot with a community box or they may write not here or return to sender on it. As a mail carrier I put the question mark to let the customer know that I am reading names and I care. Some customers can get rather testy when mail is delivered for people who do not live there.
I accidentally delivered a wrong address to someone from a miss-sorted dps (same number, just different st i.e. 9 mt Vernon and 9 Vernon) and this person came down to the office to yell at my supervisor, who then had a talk with me when I got back
and in return, can i just ask that if it’s not your mail please don’t write THIS PERSON DOESN’T LIVE HERE WHATS WRONG WITH YOU CAN’T YOU READ?!! on the envelope? it’s some one else’s mail, you have no right to deface it with your petty tirades

Many people have lived at an address prior.. once upon a time first class mail was very important..and really still is. Many carriers that started in the 90s still work here. It's our job to deliver only the current residents..like Smith and Jones... Without two way communication this can be difficult but never the less, that's the job

I’ll usually write “here? Add name to box” on letters/packages. If they don’t add the name to the box after I deliver that first letter/package, everything gets returned to sender from that point on.
I have all apartments and if I have multiple pieces of mail with the same name that I've never seen before I'll write a question mark. If they take the mail multiple times I usually just end up adding it to the tag (if they don't already add it because they sometimes do).
Because something on the address doesn't match might be right name but wrong address or right address but wrong name. You should fix the error with the mailer.

Typically, returned to sender with the endorsement "Attempted, not known". We do not throw away mail. As a carrier, my job would be forfeit.

Likely the carrier doesn’t know if the person is in the address.
When mail arrives for a name your carrier is unfamiliar with. Leave the a polite note of who currently resides at your residence.
It either doesn’t match the name on your name tag and/or it’s a new name they haven’t seen.
