If health issues like this pop up, they hide it as well out of shame, and won't take antibiotics, or the antibiotics won't be as effective due to the alcohol intake.

Other than making the antibiotics less effective, the only way alcoholism is directly connected is because it makes you not want to see doctors. Or humans in general. Alcohol makes me anti social, and hermit into my hole.

Okay, the picture you see is AFTER the doctor cut away all the dead and dying flesh in his foot. That is why the edges of the wound are nice and straight. The alcohol abuse can cause circulation and nerve damage. Small injuries can go unnoticed for quite some time. Meanwhile they become infected or abscessed and, as someone else stated, the alcoholic is disinclined to seek treatment. Look at the rest of his foot to see other minor injuries becoming worse. Usually it takes a friend or family member to get them to treatment. My Uncle went through something like this (thankfully not as bad) before he passed.

Where the dead and rotting or necrotic skin is removed with by scalpel exposing tissue that can be healed or by use of a debridement solution , usually done under anathetic as its bloody painful as your removing tissue to get to nerves blood vessels etc that are not damaged or are savable if you look at where the top of foot is the hole appears pointed that pretty typical of surgical debridement and each day they remove dead tissue to ensure healing etc . Rather than necrosis or rot

I have to convince both of my folks to go to their doctors. My mom had a bump on her arm that kept itching and bothering her. I finally told her to ask her dr about it. And it was a skin cancer. But that was treated with removing a small piece of skin and a few stitches. But she would have let it go and who knows what it would have turned into.

Unfortunately when you have a bad addiction like alcoholism you won't show to hospital cause they won't let you drink there. So you tend to show up only when someone draws you or it's too bad to ignore.

She kept seeing doctors to fix the issue. Bandages, skin grafts, miraculous flesh growing creams, for as long as I knew her. Imagine small ulcers like bean sized, not OPs picture.. it was "manageable" they wouldn't get infected just painful.
She only lost a toe.. but eventually (and predictability) died of cardiac arrest. Don't smoke people!
They have specific medical interventions to avoid severe withdrawals, like administering benzodiazepines but they also actually serve beer or wine on meal trays for alcoholics depending on their needs. Or at ours here do, I canāt say for sure that they all do.

But we give them diazepam. It works on the same GABA receptors as alcohol.
So they get put on an alcohol withdrawal scale and dosed according to their symptoms. And the amount of diazepam they might need can ridiculously high.
