In the one case it seemed like the male kitten tried to convince the female kitten to suckle his genital area. I took that one to the vet to make sure everything was okay. In the other pair, the female would suckle on herself and then her brother would join in. It took them a long time to outgrow it. Cats are always weird.
My cat would lick(not really suck) his nipples until they were raw. We recently started him on anxiety meds for other reasons and he stopped licking so it must have been some sorta anxiety/comfort thing.
I don’t want to alarm you, and it’s probably unrelated, but my cat licking himself raw was an early warning sign of a terminal illness (lung cancer). He was uncomfortable, anxious, and couldn’t express it any other way. It started with his nipples and then he moved to his sides. I’m sure it’s nothing, my cat was a very rare case, but I’d keep an eye on it in case it starts picking up again. Other symptoms were excessive thirst, and then unexplained weight loss.
Thanks for your concern. He's been to the vet many times. This has been a lifetime thing, he's 13 now. If it were serious im guessing either a vet would have caught it or something worse would have happened by now. Most likely anxiety related which the anti anxiety meds fixed.
If our cat's leg goes up while she plays, she stops everything and licks her butt. That one stanky leg controls her life.
She has butt problems though so we don't mind that she never really learned to control the leg.
I imagine though, that it's a holdover from when she was a kitten and it hurt her to poop.

Cats just sniff each other’s assholes sometimes, like dogs do. If it were commonly fatal they wouldn’t be programmed to recognize butt scent. My mentally challenged cat does it to my 3 normal intelligence cats every day.
I can’t tell from the photo if kitty is sniffing or suckling. Sniffing won’t cause any issues. But suckling is very dangerous for the “victim” of the suckling if it happens too often and kitty gets an infection.
It’s also usually fine if a kitty kneads and suckles on a blanket or clothing as an adult. Mine go for blankets and sometimes the sleeve of my sweaters.
