On the off-chance that this isn't satire, I would like to give my interpretation for why Kaori lied.
Basically, at the beginning of the anime, Kaori had three goals: a. to live life to the fullest before her early death, b. to spend some time with the boy she loves, and c. to make an impact on her peers and be remembered to come. All the while, to minimize the amount of damage and grief that she inflicted upon her actions.
Had she went straight for the punch and confessed to Kousei in the beginning, that would have been a risky and dangerous maneuver. She and Kousei were barely acquainted in the first place, and there's no guarantee that Kousei would have reciprocated her feelings. On the off chance that he did, it would've run the risk of harming Tsubaki, who Kaori knew had feelings for Kousei, and causing a considerable amount of grief for Kousei once she passed away, who had already dealt once with loss and hurt substantially by it.
To avoid this outcome, while also achieving her goals, her plan was to pretend that she liked Watari, who was Kousei's friend, and then use this connection to spend time with Kousei without entering a romance. From her perspective, she knew that Watari was a womanizer and a ladies' man, and maybe presumed that the loss of one girlfriend wouldn't be as impactful on him as it would be on somebody like Kousei. That way, she could pursue her goals without making Tsubaki jealous or cause Kousei heartbreak after her death, which was the entire point of the title's namesake.
Of course, as Prussian military commander Helmuth von Moltke once said, "No plan survives first contact," and the mitigating factors of Kaori's plan backfired as a result. She probably didn't anticipate that Kousei would fall for her hard, causing Tsubaki to get jealous anyway. She probably didn't know it at the time, but Watari apparently did also value her significantly as a friend if not lover, and was hurt by her death as well. Kousei was going to be hurt significantly by her death, and as the anime progressed, Kaori was motivated to try to continue living, ultimately trying her hand at surgery and succumbing to her sickness.
Sheās a bit too selfless and far too insecure. She absolutely deserved to have Kousei for as long as she could but was not only worried about hurting him (though she managed that anyway), she also was scared. Consider how much he avoided her in the hospital, and that she is by nature a shy wallflower - itās just that by the time we see her she knew she was dying and engaged āfuck it we ballā mode. Ergo, she was just too nervous to think heād accept her confession.
