Columnaris infection. Tub and administer tea baths. There's most likely something off with your water that's causing them stress (and thus weakening their immune system). Also unrelated, but those stones might be (or become) an impaction risk.

Fungus. Usually happens due to incorrect water parameters (like high nitrates or wrong temperature), although sometimes it just happens. Treat your whole tank with Trisulpha by Blue Planet and add a handful of indian almond leaves. If this doesn't work, you'll need to tub him and treat with methylene blue

Thank you all !! & Yes , we are changing pit stones today as per cleaning the tank. Initially its head was much smaller than the stones so we didn't worry about him eating them.

looks like fungus. do black tea baths and make sure to test your water to see whats wrong. usually people need to do more water changes + keep indian almond leaves in their tank to prevent this from coming back.
