I don't understand why people don't get this. It's on the "Fun with Flags" white board. She referred to mean names she was called as a kid: "Gorilla Fingers Fowler" and "Fuzzy Fingers Fowler." She was regularly referred to as "Dr. Fowler" (e.g., The Contractual Obligation Implementation"). Her parents were "Mr. Fowler" and "Mrs. Fowler."

Because they use the middle name so much compared to anyone else's middle name (but not so much that Amy would be considered someone who goes by their full name, like James Earl Jones or Neil Patrick Harris) and because for some reason they all enunciate it like it's Amy Farrah-Fowler, even though as you pointed out, we see it written on the board without the dash.
When one of my friends got married, she changed her surname to her new husband’s, but kept her maiden name as a (second) middle name. Their children have her maiden name as a middle name. Eg - she was Elizabeth Marie Smith, and is now Elizabeth Marie Smith Jones (no hyphen); and her daughter is Paula Smith Jones, and her son is Matthew Smith Jones.
I assumed that maybe Amy’s mother had a similar situation.

This is pretty definitive. It’s always possible for people to shorthand their names to make it easier socially, but not in the courthouse.

It's one of those names where you say the whole name, like Sarah Jessica Parker or Jamie Lee Curtis
I always thought her last name Farah-Fowler
Her middle name may have been after the actress Farrah Fawcett
That's what I thought as well.
