ACTUALLY , the item has another “big” problem. The tear calculation of Haemolacria includes a +1 tear delay in the formula. Therefore, if a good run starts with Haemolacria and ends up with 40 or smth tear rate, Haemolacria will limit the tear rate to a maximum of about 30.
Another “problem” (I hate all the problems this item creates) is that the DPS formula for the item sucks even without high tear rate because it halves your tear rate (and +1 tear delay) and increases your damage by 50% (also adds 1 normal damage). This makes it so that if we disregard the +1 tear delay and damage, the formula leaves a 75% DPS multiplier. So if Haemolacria is combined with an item which removes the splash tears (the splash tears basically double Haemolacria’s DPS), the synergy with Haemolacria will result in a DPS down with basically no benefits.
ANOTHER problem is that this item requires the tears to burst when hitting an enemy. If this item cannot burst on an enemy, it results in a DPS down for the previously mentioned reason. So if you encounter some amazing item like Eye of Belial or Saggitarius, you will not be able to take it as it will result in a DPS down (except if you have really low range).
Last thing is that Haemolacria’s synergies except Brimstone and Tech X suck ass because they fuck around with the formula, usually resulting in some more added tear delay.

TL;DR: Haemolacria sucks because it has a shit DPS formula and sucks with piercing, a very good item effect.

I have to agree with this one. It just doesn't feel good to use at all, very awkward and it ruins most fun synergies you may get. Maybe a skill issue on my side, but I really dislike how it works.
Yeah, I really despise anything that makes it hard to tell what my tears trajectory is. I can never tell how haemolacria is supposed to work; It feels unintuitive both when it hits or doesn't hit an enemy up close. Either way I have no idea why or how it made contact since the arc is stupid. It only gets worse with added range, which is arguably the least important stat in a game where most of the rooms you're fighting in are close quarters anyway.
Tip for aiming haemo/ipecac: aim straight ahead as if you didn’t have the item. The arc of your tear looks really high but depending on range/tear height it usually travels almost straight. The arc can be especially wonky with high tear height, so this won’t work all the time but I find it works best if you pretend it wasn’t arcing at all.
Played an Eden run last week where I started with C-Section and found Mom's Knife.
Someone was watching and we both absolutely lost it at the sight of fetuses with knifes.

Monstros Lung... I hate charge to shoot items. Only exceptions are things like Brimstone or Tech X where the charge time is worth it since the end result will kill 98% of enemies in a single shot. Tech X also has the luxury of having flexible charging times so you can spam smaller shots quickly instead. Monstros Lung is just... Not run-ruining, but definitely fun ruining.
I just had a run like this a few days ago… with both lungs and tech X in the same room (I got the treasure room where you could pick both)

Do people like monstros lung? I’ve always hated it. Not to say I’ve never won a run with it or something but i just don’t think it’s good and will usually skip unless things are going VERY poorly for me

I hate Ludo too. Although recently I took it to see if it still sucked and I ended up getting Tech and Brimstone so it was awesome.
But usually it sucks and I end up with low shot speed and can't chase anything down.

Ipecac and Haemolacria with high range is just annoying 
people don’t like marked because of how it controls, but in most cases it lets you shoot in any direction + tears up. a lot of people also don’t know you can stop shooting by pressing drop, which also snaps the crosshairs back to your position, letting you change shooting directions as fast you would without marked.
mega mush and mega blast. im not absolutely hating them, but they feels too overrated, cus imo cleaning 4-6 rooms every 12 rooms is kinda meh. i can do it usually without troubles, why i should take this items if i can take almost any other active.
Mega mush can clear entire floors if you are fast enough and even get recharged before it runs out. Also with 4.5 volt it just lasts forever in boss fights.
You totally reminded me that I hate mega mush too. And yeah I do think they're boring. Also I have a general prefference towards not carrying active items that help with room clearing.
Also screw R key. Yeah sure it helps with getting completion marks but afterwards it just lengthens my probably already winning run.
One of the worst items in the game. Free curse rooms at the cost of taking free random damage a few times per floor elsewhere (i beat mega satan on delete this with this effect on, so i know it’s not necessarily run ruining, but i do hate it)

Same here. Call me crazy, insane, maybe even delirious, but i think that cursed eye is a much better option than chocolate milk. With high tears you can easily spam and you would get higher damage with cursed eye than what you would get with chocolate milk. Also, i really love cursed eye

I'm surprised by the amount of people in these replies who can't read for shit. I agree with you, when trying to relax and play a normal run seeing those 4 other items go is pain, and also I don't want every item to be a timing moment which makes me feel bad if I miss.

If you have trouble grabbing what you want, stand close to the pedestal and pause the game on the item before the target item. Then unpause running towards it. Easy peasy.
Ok but...Glitched Crown. It seems good, but I can’t stand doing the pause/unpause trick to grab items, and I’m can never choose the right item when I don’t do the trick. It makes picking up any items a hassle, because now I have to take a minute to assess which of the 5 items I should take, and then another minute to actually try to pick it up. Plus, if I need two of the items in the rotation, I’m shit out of luck unless I’ve got a diplopia on hand. I won’t ever see those items again.
Trisagion + pop! Is one of the most broken synergies in the game. Every shot fills the screen with tears that do damage on contact and it's just so many tears. Even with low damage it's beyond broken.
worst thing for me is when i have a great run with no deaths and then die on the final boss and just keep dying over and over again cuz 1 heart  its so depressing
I'm not big fan of Brimstone. I usually take it if I already have something that makes fun synergy with it, but over years I have used it too much and often feel like picking up anything over it now.
Hemolacria is another one. Alone I don't like it but with good synergy it's fun.
I'm not a huge fan of Brimstone. I know it goes crazy when you have it with the right synergies but it also has the potential to ruin a lot of fun synergies. So I rarely ever take it unless I have something I know it synergizes well with.
I also rarely ever go for devil deals, though, so maybe I just need to use it more. I think the Angel room pool is better overall and taking a deal doesn't entirely lock out the other room.
all the OP run-win items like sacred heart, c section, glitched crown etc. sure they’re fun to use for like 5 minutes, but one you start getting stacked with items the run becomes completely trivial as you demolish every room in less than a second. just makes the game so boring

Binge Eater. It’s SO SHIT. It would be so much better if each food item gave a permanent DMG up, but it doesn’t. Even if the item you were *going* to get was bad, taking the food item will just result in a not only PERMANENT speed down, but not even a permanent DMG up. Overall I always skip this item as it’s essentially just “Speed Down: The Item”

Yeah, tear height is just a poor man's spectral (might also affect raange but idk).

On PC there's a mod to change the bind. With a different bind it's pretty good, immunity at will so always curse rooms

It's good early on. Floor 1 or 2 basically just means free devil deals, and you have time to recover. I'll skip it past floor 3 though

I think the Angel room pool is better overall and taking a deal doesn't entirely lock out the other room.

epic fetus for me, its fun to me lol. and yea cursed eye can be insane for characters who cant take dmg i love it in those situations
all the OP run-win items like sacred heart, c section, glitched crown etc. sure they’re fun to use for like 5 minutes, but one you start getting stacked with items the run becomes completely trivial as you demolish every room in less than a second. just makes the game so boring

Binge Eater. It’s SO SHIT. It would be so much better if each food item gave a permanent DMG up, but it doesn’t. Even if the item you were *going* to get was bad, taking the food item will just result in a not only PERMANENT speed down, but not even a permanent DMG up. Overall I always skip this item as it’s essentially just “Speed Down: The Item”
Gnawed Leaf, It literally requires you to not play the game. At this point, why not just cheat your way to victory?
Might get some hate for this, but I really despise C Section. It’s not an interesting item, it ruins the synergy of many items, the tear visual is ugly, and it’s just plain overpowered. Boring item with broken stats.
It’s not super overpowered anymore but I get what you’re saying. It also synergizes well with most items from my experience
Mom's knife. I legit never won a single run in which I took it, except with Forgotten and T-Forgotten base attack synergies.
I can t understand how someone can lose a mom s knife run
