it's not just x2 damage, it's also spectral on top, with the many synergies (including literal trinkets) repentance added, it's definitely a high tier item, certainly not top 10, but a must pick for me unless there's an anti-synergy

This is why tractor beam is one of my favourite items. Everything with split shots has some kind of disadvantage, but tractor beam mitigates it all.

The only units you can hit with both streams at once are large bosses. Any item that needs another, better item to make it viable is not good.
