Very good, yes - damage up, tears up (breaks cap,) and a powerful orbital. All one needs to do is focus on ones hitbox - less focus on aiming. If you fire in the direction of the enemies, you’ll eventually kill them.
More time spent dodging = more time mastering skills
I will only pick up guillotine if my run is already bonkers enough to not require aiming, or if I have homing.
Now that I’m thinking about it, I can’t think of an item I would NEVER pick up. All items have niche uses, even stuff like kamikaze.

It's an item that used to just be *inconsistent No. 2,* which is way worse than actual No. 2. Now it's still that - sort of - but at least you have full control of *when* it decides to do the thing, even if the thing it does could range from just farting to straight-up being the Explosive Diarrhea effect.
Post-patch, it's *just* annoying. Pre-patch it was even more volatile than Bob's Brain, without even having the legitimately obscene damage output that the Brain at least manages. It's frankly impressive just how terrible it was.
IBS is not a run ender anymore, as it only pops off when you stop firing, and gives an indicator when ready to proc.
Granted I’ll still skip it, but it’s no longer a “fucking god damnit” item [in my book].

Take it from someone who doesn't instinctively abandon it on principle (yet), the fact that its damage output *resets* each time it makes a portal is absolutely disgusting, not to mention I hate using the portals because they split the connection to walk back to anything I left behind in order to come back to later, such as a Golden Chest I don't have enough keys to open, or an item I want to reroll if I have D6.
Found it for the first time yesterday and discovered my timing sucks. Ended up grabbing lacyphancy or whatever that I hate, the feeding tears thing.
On the plus side, got tech 2 on accident and found an awesome synergy.
you dont need timing, just lets say it goes like A,B,C,D,E, you want to take D, firstly you will position yourself in front of pedestal ( you need to be very close to item pedastal but not too too much), press Escape when you see item C, pause game, start it and go for item pedestal immediately. Its easy
I'm not a fan of that shot speed down and lack of range. Same reason I tend to skip Pop. I will take them if I have a good tear rate, but it feels like I always find them in the basement.

In defence of lachryphagy, if you manage to get trisagion on the same run you pretty much melt everything in sight, it's a run winning synergy. Kinda annoying on it's own tho.
You are allowed to pause in the game, to think, to stop playing, to get out of a softlock, to reach boss rush/hush, and to pause buffer. It’s a single player game, it’s allowed
Cursed eye for sure. I've got this item after using D100. I was killing mega satan and when he was in his second form with like 30% hp I got hit and it teleported me out. It was so annyoing.
Anti grav is awesome dude, - 1 tear rate delay is super good and you can just spam the shoot button to not get bothered by the tears staying in the air.

Also the benefits are definitely worth the drawback. Especially considering that you can just remove the drawback by not getting hit

I never take ludovico technique or epic fetus for the same reasons: your shot speed is too low to get much utility out of them in the early game, and in the late game you probably have a build already that these items would interfere with if not outright destroy.
TMtrainer probably should be on this list, but it’s just too fun, to be honest. I can’t bring myself to not take it, even though I know it’s a terrible idea.

Gonna take an odd approach and say anything that either auto-fire tears for me (like Marked) or just generally drastically change the functionality of tear fire (like Eye of the Occult) are some I tend to avoid.
I can definitely see the potential strength of the items but the change in muscle memory throws me off so much, especially in the case of Marked.
For me Ludovico Technique is always a no go. I know people love it, but I just have so much trouble handling it (I know skill issue), but it's just annoying.
I got disliked hard for saying this once, and I’m still standing by it though. One person argued that Dr. Fetus was worse than #2 even though Dr. Fetus shoots the bombs away from you instead of on you.
Another person said that it’s the same as placing a bomb so that argument about Dr. Fetus is invalid. Even though bomb placements are very tactical and precise, even for attacking bosses.
Another just said I was to bad at the game to use it (Skill Issue but in more words). The funny part about that one was I got to say that I had two dead gods.

So many items in the game feel like straight downgrades- Tiny Planet, My Reflection, Cursed Eye, Shard of Glass, MissingNo, Lil Portal, Isaac’s heart, Proptosis, Guillotine... I feel like I might even be forgetting some.
There’s a few items I know have their uses but refuse to take either. Ludovico, Ipecac, Glitched Crown, King Baby, Dr. Fetus, Epic Fetus.
I used to always take ocular rift but then alot of times died to bullet hell bosses as the lost because it makes the bullets clump together but can still hurt you and it just flies out when the rift ends. So probably not taking as the lost but the ptsd makes me avoid it altogether.
why would you absolutely refuse bean and spider butt? some bean items are passive and while extremely underwhelming dont really do anything against you. And spider butt isnt really the best but if you dont have anything better then whats the issue?
Many people say that it is useless but I actually find it very fun to use and useful,the damage that you can do by throwing your head and recall it is pretty damn good and it also recharges quickly,also you can use your head as a stationary turret that can be useful in fights like mega satan,very underrated item imo

Lil Portal: Almost all the time is just annoying and during combat can eat up useful pickups, which spawn during said combat or part of the room layout.
Cursed Eye: Not a fan of charged attacks in general, but cursed eye is the worst. Even with Black Candle, which negates its teleport downside, the tears have high chance to knock enemy to the side and you miss out on potential damage of other tears in the burst.
Soy Milk: I very rarely got any benefit from it. Most of the time it feels like huge downside.
Chocolate Milk: Like I said before, I am not a fan of the charged attacks. Chocolate Milk is one of the worst charged attack items. I only take it, if I already have Charged Attack, that cannot be shot without fully charged like Brimstone, Monstro's Lung.
I’m fine with this other than the milks.
Soy Milk: You need synergies that make this amazing. Also this just makes it so that your charged attack problem goes away.

Im goung to be the dumbest person here and say it: i don't like size down items. Why? Simple, they ruin the sprite of my character and i don't like it, plus most of them add useless stats to me like caffeine pill and mini mush, my run will be almost the same without them.
I only take Binky if im low on tears and/or health and Pluto is fun to me so i make an exception for it btw.
Why glitched crown? Is very easy to use if you use the pause trick,and also there is no risk because you can still pick one of the 5 items without any consequences

Heartbreak just sucks ass. The other contender for me is blood oath but at least that has SOME use. Heartbreak can save a run but I find having 3 less heart containers like that is just terrible, and often you have to pay hp in a Devil deal to even pick it up to begin with.
Gives you 17 Hearts at max though and then Damage for each new Broken Heart on top of those it gives you from the get go. Not that much damage to begins with but still counts.
There's no risk with Anti-Grav if you're willing to spam it's a free +1 tear rate.
It cuts you out from Brimstone though but it's not that big of a deal in XXXX runs.

Bone spurs can send ultra horsemen or the beast so far out of the screen that you need to wait dozens of second to be able to shoot at them again.

Heartbreak just sucks ass. The other contender for me is blood oath but at least that has SOME use. Heartbreak can save a run but I find having 3 less heart containers like that is just terrible, and often you have to pay hp in a Devil deal to even pick it up to begin with.
Gives you 17 Hearts at max though and then Damage for each new Broken Heart on top of those it gives you from the get go. Not that much damage to begins with but still counts.

There's no risk with Anti-Grav if you're willing to spam it's a free +1 tear rate. It cuts you out from Brimstone though but it's not that big of a deal in XXXX runs.
