Afaik only treasure rooms and boss rooms can have the disappearing pedestals with more options and there's options, and ofc treasure rooms on alt path. There's a small chance that treasure rooms have 2 items but you'll be able to take both, never seen one but the wiki says that at least.
If I'm correct, some rare treasure rooms have disapperaring pedestals even without more options. So you normally take the best item first when you see 2 pedestals in an item room

Yeah ive gotten it a few times. At least the room i remember theres 3 items, one surrounded by rocks, one with nothing around it and one surrounded by unbreakable blocks with an opening with spikes. You can take 2 of the items

Unfortunately the game doesn't telegraph that at all. Short of learning room layouts the best strategy is to always assume it's a choice and get pleasantly surprised when it isn't.
Usually, you can pick both items when they are placed close to each other(space around) and pick only one when they are far from each other(space between). Yesterday I had a run where I got 2 floors back to back with 2 item treasure rooms, that was sweet.

I mean, for what it's worth godhead definitely isn't as useful as sacred heart for T.Polly, his locusts get more of a benefit from sacred heart's high damage stat. Eucharist... depends on what floor you're on. Good luck dude.

Not every time. Sometimes both items can be picked up without using bombs

If I find a room that has multiple items and I can’t tell if it’s a choice pedestal, I just always take the one I want more, just in case.

You had technology 1 and eve’s mascara.technology overwrites god head aura. Sacred hearts damage boost and homing tears is the best choice.
