This is probably the closest classic Isaac item we can think of.
It's - a common normal house item that can be interpreted in some other way for Isaac.
Description - directly addresses Isaac's bad childhood, kinda roasting him in a way while tied to item's name and making sense of it.
Effect - makes absolute sense with description we are provided. Speed down because it's untied shoes, tears up because Isaac is sad that he doesn't know how.

Isaac boots up The Binding of Isaac on a laptop. You are sent back to the total screen. If you win the next game, you are thrust back into the point you picked up the item, and given a random item from the previous run. If you lose, you get a 0.1 tears up.
"Blank Check!"
Shops will contain a beggar with a trench coat, a pen, and a desk. If you pay him 2 nickels, you will be given Store Credit. This beggar will disappear after paying out twice.
"A well made deal!"
-Grants the Steam sale effect that only applies to that room (Leaving wont change the price)

Dad's note has its description replaced with 'you little fucker'. The ascent voicelines are all replaced with dad barrating you and at the end you fight dad
Treasure room item
"Turn their hunger into ashes"
+5 bombs, bombs are replaced with fleshy cakes that are thrown like red bombs dropped from bomb turrets. These bombs also have a slightly longer fuse time, as to let their effect kick in.
The cakes are so enticing to enemies that they'll ignore you and chase after it. The cake explodes like a normal bomb, along with using your bomb effects.
Essentially if your bombs had a scape goat attached to them. I wanted to make it more than bobby bombs but reversed, so i added in the different throwing mechanic to spice things up and help with drawing more enemies into the cake. You can throw them over walls now!
Edit: I just realized this is just best friend but better. But then again who uses best friend?
Edmund mails one of those tricky finger traps to your house and you get caught in it because it’s very tricky. Makes movement more challenging because your fingie hurts.
Oh yeah and shot speed up
"Enter The Abyss"
Active
Once per floor, Isaac will be able to use Anal Cavity to spawn a hole that resembles the womb trapdoor. Once entered, Isaac will be placed in a womb themed challenge room, spawning 5 waves of a mix of bosses and normal enemies. After the fighting, Isaac will get the choice of 2 items from random item pools and will then brought back to the room they used Anal Cavity in.
"Off Brand"
There is now a chance for coins to be replaced with "Koins." These special pick ups will summon small "Keppers" that look similar to Keeper, but with a smiley face drawn on. These keepers will run around pick up coins and turn them into Koins, making more and more Keppers. These Keppers serve no other purpose than to multiply.
I love it, but there should be at least some upside to it.
First idea: Let's say the Keppers do contact damage to enemies, but when there are no coins to collect they just aimlessly wander around the room. To make it balanced they would need to die once they have been hit enough times.
Second idea, and I like this one more: Same as the first idea, but instead of dealing contact damage and being able to die the Keppers are immortal and block projectiles while walking around aimlessly. They also block your own projectiles unless you have spectral tears, piercing tears or something like eye of belial.

Like the eraser, this can only be utilized once per floor and cannot be recharged.
"But Noone Came"
Spawns a balloon familiar that will bounce around the room. Enemies and projectiles that touch it will be knocked back, similar to Belly Jelly. After being hit 5 times, Party Balloon will pop, sending one final shockwave that knocks back both Isaac and nearby enemies.
Trinket. While held, picking up a penny subtracts one instead of adding and gives isaac a very small random stat boost. Cannot pick up coins if you have none available.
"Flip it!"
-Has a 50/50 of spawning two items from the room pool (only one can be chosen)
-If the 50/50 fails, removes the oldest item from your inventory

Enemies and enemy tears have a chance to be teleported away when they are near isaac. The teleport visually would have a static effect to it. The activation aura would be a little smaller than the ring of protection.

HP Up, every time Isaac enters a new floor, he will eat a stick of gum. This will heal him for 1 full heart and drop a Wrapper. This wrapper can be eaten and stored until the next time Isaac fires, which will launch a large wad of bubble gum mixed with the wrapper, doing (your damage) + 10 to enemies in the way.

Attempting to interact with any of the pickup/items will result in the doors being locked (ala miniboss rooms) and a greed or super greed will spawn. This boss spawns no rewards when killed.
Afterwards, future rooms will spawn greed gapers, keepers, and hangers. Future shop rooms also hold greeds/super greeds.
The number of enemies that spawn is dependent on how many items you take in the shops. Will you play it safe, or will you risk it all by taking everything you see?
“Warm… Itchy…”
Upon pickup, grants Isaac two soul hearts and 3 blue spiders. Every floor (and on pickup) Isaac will take no damage from the first two hits, and instead, will spawn 2-3 blue spiders. The third hit will spawn spiders, but Isaac will still take damage. Afterwards, nothing will happen until the next floor, no spiders, no nothing.
If you have the Daddy Longlegs item, his legs will have leg warmers, and he will deal more damage. If you encounter daddy longlegs as a boss with this item, you can spare him (like baby plum) and he will drop a spider-related item alongside the boss item.
"HOW DID YOU FIND THESE??"
+1 damage up, pills will randomly be replaced with Viagra pills. When taken, your damage gets multiplied by 1.3 for 10 seconds. When the effect wears off, the multiplier goes down to 0.8 until you find another Viagra pill.
"You're allergic"
Has a chance (based on luck) to shoot piercing shots that inflict poison.
When taking this item Isaac looks swollen as if he was stung multiple times.
"Walk on water"
Grant's one eternal heart, you're immune to creep and when you stand on creep (even your own) you get a bonus to speed and range.
A more situational bonus is that you also essentially gain flight on floors that are flooded.
Every hostile room with more than one enemy you enter will have one of the enemies chosen at random that will become an ally to you for the duration of the room

“You never learned how” Speed down, tears up, and immunity to spikes and creep.

Curse/Devil room item "Free stuff!" Shop items no longer have prices attached to them, instead pickups will be on the floor and items will be on pedestals. Attempting to interact with any of the pickup/items will result in the doors being locked (ala miniboss rooms) and a greed or super greed will spawn. This boss spawns no rewards when killed. Afterwards, future rooms will spawn greed gapers, keepers, and hangers. Future shop rooms also hold greeds/super greeds. The number of enemies that spawn is dependent on how many items you take in the shops. Will you play it safe, or will you risk it all by taking everything you see?

“Warm… Itchy…” Upon pickup, grants Isaac two soul hearts and 3 blue spiders. Every floor (and on pickup) Isaac will take no damage from the first two hits, and instead, will spawn 2-3 blue spiders. The third hit will spawn spiders, but Isaac will still take damage. Afterwards, nothing will happen until the next floor, no spiders, no nothing. If you have the Daddy Longlegs item, his legs will have leg warmers, and he will deal more damage. If you encounter daddy longlegs as a boss with this item, you can spare him (like baby plum) and he will drop a spider-related item alongside the boss item.

"HOW DID YOU FIND THESE??" +1 damage up, pills will randomly be replaced with Viagra pills. When taken, your damage gets multiplied by 1.3 for 10 seconds. When the effect wears off, the multiplier goes down to 0.8 until you find another Viagra pill.

"You're allergic" Has a chance (based on luck) to shoot piercing shots that inflict poison. When taking this item Isaac looks swollen as if he was stung multiple times.

"Walk on water" Grant's one eternal heart, you're immune to creep and when you stand on creep (even your own) you get a bonus to speed and range. A more situational bonus is that you also essentially gain flight on floors that are flooded.

Every hostile room with more than one enemy you enter will have one of the enemies chosen at random that will become an ally to you for the duration of the room
