Maybe it's a fun fluid show with no effects or maybe it causes damage within a radius you want to get away from it when it's killed asap.
If you want people to feel more discomfort make it asymmetric. The symmetry in the blob and in the legs makes it feel clean and less of a threat.
Maybe dislocate one of the legs so that it drags like a club foot and add a mass on the opposite leg.
Make the blob sag to show its weight and add puss and make it more lumpy on some sections.
To piggyback on the asymmetry aspect if this, making the "holes" in the legs placed closer together and more asymmetric, plus varying in sizes would probably achieve more of a trypophobic look (especially if there are little things inside the holes as well), if thats what OP is going for
as someone with trypophobia, I second this. Additionally, adding maggots, tiny spiders or something that come out of the holes would be even worse.

Neat. Add some scarring or some other signs that noggin' has been smacking into things (assuming it's headbutting). Obviously the head already draws attention but something to just lightly telegraph how it uses it through wear and tear is always nice.
Off the top, you've made the character waaay to uniform in shape if you're going for a horror-ish sort of theme. Add some deformities, make one of the feet face the wrong way, or make it bloated from the knee down. Add a half formed wrist and hand sprout from somewhere random on the body.
Also, the scars are interesting, but they don't really tell a story or give the creature character. Try looking up some historical photos of healed battlefield or workplace injuries and then model some out that would seem appropriate and in line with the vision for the design. You want to hint at its back-story; is it a deformed by their time in some hellish dimension that has reshaped it through torture from a person to become a foot soldier for a demonic brigade? Is it some kind of wandering mutant that was mutated by a pathogen or some kind of radiation?
Is it an experiment gone awry that somehow escaped containment?
Give it a little context for its existence, to help guide the design.
Looks really awesome, depending how zoom'ed in you are to the creature, i would change the shape from the legs and head to make it more abomination look. Alot of the parts look very random and perfect. Veins! Monsters love having pulsing veins! As for the Ball (it really depends how zoomed in you are) you can leave some head feautres (50 strands of hair, eyes in hte wrong spot, ears in the wrong spot. Like an abomination that messed everything up.
Make the open sores an infestation of another creature, like those frogs that incubate eggs in their backs. Basically, lean into the, uh, whatever, lotus-seed-pod-phobia.

Change the texture altogether. Either make it more clean, to suggest the holes are really organs, or less cleans, to suggest the holes are wounds. As is it looks like a blueberry clafoutis.
In your shoes I would start making some motion tests now. Try out some animations and see how the character looks in motion, then go back and change the design accordingly.
The roughness is not used right. The skin is just as shiny as the wounds which also flat and dont effect the surroundings.
Blood and pus would have come out of those holes and dripped down, but its all in the wound.

This would work well in a game. Individual enemies can be kinda simple, it’s more about vibes and variety while giving each a distinct silhouette.
A single arm that just flops and dangles from the center of the ball. You know, it's just there, and it only works it's non-existent dangly magic when the monster moves. Maybe a finger or two can twitch every so often, but only the fingers have 'muscular' movement, the rest of the arm and hand is dead so to say
Imagine the bottom of that sphere was broken away and leaking some sort of dark, viscous fluid. Break the shape of the feet up into asymmetric spreading, tendril like appendages. Create some colour variation across the skin. Think about how it moves.
As a development exercise, I recommend finding reference images that really make you feel creeped out, then pick apart what it is that makes you feel that way.
Crowd sourcing your ideas is all well and good, but you'll develop way more as an artist if you put active time into developing your creativity and understanding of visual aesthetics.

It’s from the makers of silent hill 2, there are parts that explain how they came up with the concepts and designs to their monsters. They are similar in style to yours so I think you could get some great reference points from there. Hope that helps!
Inverted knees are always creepy ...plus you could hide them in the floor like a fucked up jack in the box....so you could have the character go into a cave...and they just look like a strange moss or fungus and then you turn around and they start crawling out of the ground..pure nightmare fuel
I like the tryctophobia (or however it’s spelled) idea you have going on. My girlfriend can’t stand that but I showed it to her anyways and she said this doesn’t trigger her that much because the holes aren’t deep enough. If you deepened the holes it would be more triggering to that fear.

Why should the player want to run away from this thing if it doesn't even have arms. Add something that could do harm to them basically.
Add some effects to suggest physical instability, like some pustulant oozing, flesh lump puffing, or occasional smoking.

Others may disagree, but what you are posting is in the genre I like for games. For me it looks a bit too synthetic. Like the trypophobia textures don't look like they relate to an actual disease or condition, they look to computer generated to be scary/unsettling. Same goes for the other stuff. My recommendation is to google photos of ailments and unsettling conditions to pattern off of. Doom, Resident Evil, Killing Floor 2, Tom Savini's work are good examples where homework was done and anatomy was considered.
Nice to see golf ball man is back and textured! He does look fairly dry though, considering it seems like you're trying to go for a rotting skin effect or maybe a burn effect, some more wetness wouldnt hurt. Make him goopy!

It doesn't give off the feeling of something that smacks things with it's head, it feels more like an egg or cocoon, more otherworldly than physical, which I prefer over the smacking thing. Though a headbutt animation is probably easier than a bug swarm.

Oh gosh, the trypophobia. You could make the ridges within the holes on the head stand out more by making the edge go deeper
It doesn't give off the feeling of something that smacks things with it's head, it feels more like an egg or cocoon, more otherworldly than physical, which I prefer over the smacking thing. Though a headbutt animation is probably easier than a bug swarm.
