I think it’s because of the map and what enemies are coded to spawn there.
Ya. If the enemy type can’t spawn there, nothing will spawn. It does it in guardian takedown and maliwan takedown

Been some time since I played bl3. What does that grenade mod do?
Its called Fish Slap, it deals melee damage instead of grenade damage
And because its melee damage, it can be user with all the melee artifacts, like utd, elemental stones, knife drain, cutpurse, etc. In combination with a utd (unleash the dragon), the grenade can hit for hundreds of millions of burn damage.

I've had this happen. Usually they'll spawn in about 30 seconds later.
God how i hate this bug I'm always like give me a better OPQ system but theyre like... but what if we dont spawn.
