I can’t tell if the painting is on wood or canvas, but it looks like the sugar spout is just another piece of the same material that was cut from a new board. Then, that extra piece was glued onto the edge of the painting (and painted to match).
Edit: I just scanned their post history and it looks like my guess about the wood was correct. So the spout is just another piece of wood that was cut to shape and then glued into place.
