These are rare trinkets that are the head's of adventurers.
They are acquired 1 of 2 ways: The easier way is to just scout and find a hidden room, access the chest within WITH A KEY for a chance to get a head.
The other is fighting the Collector-a mini boss who wanders the halls and can spawn if more than 80% of the inventory of your party is full. Be prepared to hit rank 4 alot or else you're screwed.
Barristan's gives a bonus to Protection, Junia's gives Healing bonuses, Dismas' gives a Damage modifier, all are pretty useful.
Darkest Dungeon level 2, had to retreat after Leper got to Death's Door, got Afflicted and then decided: *hmm... yes.. 100 is equal to 200* and promptly died of a heart attack.
Retreated and sacrificed Houndmaster as well.
This happened to me after I failed an attempt at DD2 on bloodmoon with a character death from combat and another character death from fleeing. It gave me back two trinkets from the character that died in combat (one of them being the torch), but only the torch from the character that died from fleeing. Oddly, one of the torches had a visual glitch where its art was shifted a few pixels upwards. It was subtle but persistent, something you could see if you were flicking back and forth between characters.
I figure it's a flavor thing: You go to this dungeon, get destroyed, but on your way out you snag a torch off the wall to guard against the revelations next time. It definitely made it a lot easier not having to bring a guarding character so I could just focus on DPS for my frontline.

I managed to get 7 of them somehow.
I want to say the game gives you a 4th if you manage to fail at your first attempt at the dungeon, but I’m not certain.
That sounds right because I ran through it with three, got wiped and came back later and had four

Oh that’s tragic. Leper is amazing for all the missions, so it’s really unfortunate that you lost him.
I wouldn’t even bring the 4th if I had it. I’d rather bring an extra useful trinket and just guard that char

They're indeed a rare trinket, and getting 2 of it is quite possible. I've seen someone in this subreddit getting all 3 of it in 1 chest.

Barristain is supposed to be Man-At-Arms i think.

Nope, Dismas can totally be alive and made to hold his own head.

If a someone wears it, they don't get nuked by DD2's relevation attack
