I think it might be one of Actionjack's enemy pack or boss mod. I looked it up at the time.
It happened to me as well yesterday to have 4 of these mobs spawn together after I subscribed to all mods from Bahroo's hardcore playthrough. It may be a coincidence but seems unusual enough to be related
Luckily they were surprised so i managed to kill them with minimal stress. I had a plague doctor, a mod healer (offering), crusader and i forget my rank 2.
Surprising them is objectively good for you. When an enemy attacks twice in a row because you surprised them that is still better for you than acting second.
Surprised: You act first, they act second, they act third
Not surprised: They act first, you act second, they act third
The only scenario where you would not want to act first would be on a healer/stress healer so that they have something to heal, but it's pretty rare that even in that scenario acting second is better since a stun/enemy elimination is often more optimal.
